USF/LIHEAP Requirements

**INCOME GUIDELINES BELOW COME INTO EFFECT OCTOBER 1ST, 2022**
Eligible clients may not exceed the following income criteria per household size:

In Order To Be Eligible For USF, PLEASE NOTE: Your total gross annual household income must be less than or equal to 400% of the Federal Poverty Level (FPL) (see below), and 2) You must also spend more than 2% of your income on electric service or more than 2% of your income for natural gas service. If you heat your home with electricity, you must spend more than 4% of your income on electricity.

In addition to filling out the LIHEAP/USF application in its entirety, eligible households must provide COPIES of the following documentation:
1. Social security cards for all members of the household
2. Proof of four consecutive weeks of income for all household members 18 and over.
(Income includes, but is not limited to, employment pay stubs, unemployment, social security, SSI, pension, business income, rental income, food stamps, TANF, etc.) If a household member, who is 18 years of age or older, does not have an income they may fill out a zero income form or write a letter stating that they have NO income which must be signed and dated by the household member.
3. Proof of residence:
If you own your home: a copy of the current mortgage statement, tax bill, or deed.
If you rent: A copy of your current lease or letter from your landlord/tenant verification form.
4. Current utility bill:
A copy of your most recent utility bill: gas, electric, or deliverable fuel must be provided. Please provide the full first page of the bill as we cannot accept bill stubs.
If you are applying for cooling benefits, you must also provide a letter from your doctor stating the medical necessity for needing to have air in the summer. The letter must be signed and stamped by the doctor in order for it to be valid.
